seen it before - reason is very simple:
YOUR PHONE HAS BEEN TAMPERED WITH
you can repro this by changing the IMEI with those DIV tools...
problem for the bad guys is that the original IMEI is still stored in secure GDFS area, and that is the one checked for upgrades!
so I suspect your phone was stolen... get rid of it!
I would not be surprised that if the IMEI is changed back to the one on the sticker, your phone is blacklisted again
