I have heard of people who have managed to get an upgrade 12 months into their 18 month contract so I think it depends on the operator.

take my mother for example, she upgraded in july to a 6233 on a 18 month upgrade but can upgade again already....
i've had my 18mth contrat for a little over 4 months and it'd cost me £150 to upgrade now...
i expect to be able to upgrade in about march time..

As part of Orange loyality scheme, you can upgrade early, the normal these days is to upgrade to an 18 month plan, also in the uk in doing so you also get a line rental discount. Most people these days keep their mobile for about 18 months before changing...well those over 30 tend to the younger set usually change every 12 months or when the latest fasion or tech phone is launched.
The network would not be too happy about upgrading after 3 months, becuase they would not get back the cost of the original phone.

Regarding 24 months contract:
It's been around in Sweden for quite a while... I bought my Z600 on a 24mth contract (+100 SEK/mth) two and a half years ago, and just recently got my P990 on the same deal... I'm a poor student that can't afford a new phone every three months.
My Z600 was meant to be locked and branded, but the queue was so long that my provider seems to have forgotten to change the firmware after getting a new stock.
The P990 was bought unlocked and unbranded though (from a third party store that could fix my contract too).
