QT Pro settings that work... Start with 3GPP (Mobile MP4) file format defaults, then change:

MPEG-4 video
176x144 QCIF image
Preserve Aspect Ratio using: Letterbox
Frame Rate: 15
Audio Format: AAC
Channels: Stereo
Output Sample Rate: 16.000 kHz
