F4:12132 This is known bug for R1ED or earlier that I also had. Also it may be the bug of EROM as well (I am not sure about this). But I updated my EROM to R3A016 and then upgrade my firmware to R1GB and this bug dissapear completely from my phone. I believe this F4:12132 is actually a code that is used by SE in regard to the menu text that it the phone uses (maybe a primary key table for SE menu text).

As others said check your EROM (update to R3A016) and your firmware (update it if you still on R1ED or earlier).

Once I got my phone infected with this 'virus'. The symptoms were duplicated folders and disabled deleting function (read only attribute). I formatted my M2 and fully flashed the phone. Eversince, I've relied more on bluetooth to transfer files as it is safer due to pairing and confirmation. I believe it's kinda local viruses commonly known with .scr extension. A lot of PCs at my office got bricked because of it and once I plugged my K800i to it, the disaster happened. Be careful then. It won't damage Java OS itself but it does brick the file manager system on M2 and the internal phone.

I think its a virus on your computer trying to copy it self to any drive attached to it so its copied it self to your phone or m2

sounds like a combination of virus (?) and obsolete firmware. you got it tough, mate . a flash would be good, and easy...

tnx for ur advices....well, i tried wat u said. i tink the codes didnt pop up anymore after but the folders are still in failure....Is advisable to reformat my fone to restore its normal operation? and if so?, how will i do it? tnx

Operation failed may happen also if you don't have necessary certificate in the phone. Check your phone security certificate. It should have few certificate. Well, I got Z610 that is unable to install anything from SE Fun&Download. I fixed it by complete reflash (can only be done by authorised service centre or paid service like wotan/kulan/DVT).

I will try Master reset and reflash EROM (use wotan/kulan it's free) and firmware to newest level using SE Update Service (SEUS). If still no joy then you may have to bring your phone to authorised service centre. If you are out of warranty then you may try paid service like wotanserver that completely reflash your phone (all contact & apps will be lost). I think I have read that someone fixed Operation Fail problem with complete reflash.
