Something fast with no lag, decent camera with decent video quality, and finally a 3.5mm jack with excellent audio quality. Not too demanding

In my studies, I've had to create a survey too, and (like you) I found Google forms sooooo much better than anything else. Your questions are good for most consumers but for Phone Enthusiasts as we all are here, some adjustments could be made. For instance:
Many of us here have more than one phone that we regularly use.
iPhone OS was a curious omission from the smartphone OS question.
A question relating to primary usage (calling/txt/multimedia/social networking/data) would help put the data into context, ie: 50% of txters plan to hold on to their phone more than a year but 40% of data users plan to upgrade within 6 months...
