IIRC, when I clicked on the K700i on this page:

the survey came up in a popup automatically. It won't do it for me a second time, so I can't post a URL. Make sure you don't have popup blockers on when you click on it.

The survey is a pop-up window, which appears when you select the K700 from the products page.

I made sure I let them know what I thought of the battery life.

its good that SE is doing this. there was also one about the Z600 and T230 some time ago.
i think the T230 survey lead to the T290. most people loved its design.
