maybe your ms is corrupt. I had a simular problem. I used a 9in1 reader, the more I swithed the duo back and forth from the reader to the phone more and more files disappered.. funny. Eventually there where only a few files visable, though the duo was full (128mb). I thought the duo was ruined, but a format fixed it. I had a backup of thrfiles so no problem. Just did a restore.
Anyway - I used a 9in1 card reader which can read ms duo without the adapter. I found that the fit was not 100%, so it was possible to insert the duo a littel askewed. I guess the data was corrupted when not fitted correctly. Now I use the ms adapater before inserting in the reader (better fit). no more problems.

each time i wanna plug it into the pc i use a 8in1 card reader. I did use the adapter. I guess there's no way I can retrieve back my data...
1 more question, do I have to shut off the phone before removing the ms or just plug it out just like that ? at least that's what I always do ...

The MS Duo should have no problem being removed when the phone is switched on. I have a problem previously with missing files using a multi card reader, again without the adaptor. I would strongly recommend using the adapter to anyone using a card reader. The Vaio laptop I now use as a Memory Stick slot built in and I find that more reliable.
On a more positive note when I got my MS duo pro from Lexar thier was info about a program to recover lost data on memory sticks that they do. Unfortunatly I don't have the packaging now but somebody else might or maybe a search on the lexar website?
