The camera flash problem is a software bug, hardware fault, or a combination of the two.
This is NOT the way it should be!! The phone would not be a step backwards if it worked as designed.
I am surprised how many people are affected by this, and how it managed to go unnoticed during product testing. Were testers just assuming it was their fault the camera didn't flash and not investigating further?

I wonder if it's just certain production batch that have the flash fault as not all K850i's have this problem?

Well I haven't been too updated on the K series handsets ever since I jumped onto the P1i bandwagon, but it appears the K800i making some good progress with modded camdrivers, flash menus, and such, much like how I modded my K750i quite a long time ago.
Other than that, if SE says it's coming out with an update for months to come, and the handset remains plagued with this flash sync problem, I might as well get a K800i to substitute!

Change the way you guys hold your phone... holding on only the side of the phone. Then take a flash picture. Also ensure that the light sensor is not chalked up by dust or other particles. If all else fails, send for service. Good luck.

I have the K850i - EU (europe) version and haven't found any of the bugs posted here... been playing with it for 2 days...
(PS: I'm using HPM-82)
- Phone can crash whilst in standby mode, requiring a hard reboot (taking the battery out) --- NOPE
- Alarms only produce three beeps on some phones rather than play the selected ringtone. --- NOPE
- Play/Pause and FF/RW controls on HPM-82 do not work ---- NOPE
- Phone can crash when using HPM-82 ---- NOPE
- Some Video playback is out of sync with the sound ---- YES, My Family Guy episodes are sometimes out of sync, but the catch back up...
- Corruption of the filing system can occur affecting photos, applications etc ---- NOPE
- USB photo transfer can lead to duplicated pictures / removal of other pictures present on the phone --- NOPE
- Video is of lower quality than the K800 --- Didn't own a K800i, but the video is of pretty good quality, maybe more poor than k800
- Occasional second or so delay between the vibration of an incomming text/call and screen illumination. ---- YES, slight delay...
- Audiobook functionaility cannot be accessed
- When you connect the phone via the usb cable, You cant see the ringtone folder ---- YES
- Phone doesnt support some Mp4 files and doesnt support some jpeg photos. ---- NOPE
- The flash doesn't always sync with the image capture, leaving dark photos in night conditions. --- NOPE
- On some phones the shutter ocasionally fails to open --- NOPE
- Not all symbols are possible to enter in the email client settings, e.g. not allowing you from adding a : for gmail addresses. --- Didn't try
- The zoom buttons both zoom into a picture and change the volume if you are listening to music whilst browsing pictures. --- Didn't try
- On some phones, adding a birthday to a contact does not give you a chance to add it to the Calendar. --- Didn't try
Seems I have a good k850i in my hands compared to some others...
What does seem to be a problem is that my pictures look "greaced"... they aren't sharp at all!

I have tried different settings, but can't really figure something out.... *pictures only taken at night so far...*
Can somebody help me out?

Not all the time. Sometimes it fires early, but in most cases on the handset I just gave back it didn't fire at all. I turned the camera around to check!
The flash fires okay in the engineering menu (discharge flash) so I am pretty sure the flash is fine - even when it doesn't fire at all in camera mode. I wonder if it's a faulty sensor, although the fact the EXIF data says the flash was on suggests not.
It's a major problem though. A flagship camera phone, that is more camera than anything else before it, and it can't take photos in the dark despite having THREE super-bright LEDs and a Xenon flash!!

To everyone with camera problems:
You either have a defective model or you're blocking the sensors when shooting. Your fingers must not stick out from the back of the phone, even one centimeter is enough to screw up the picture. It took me some shots to get used to it but to SE's defence, they do give a leaflet with detailed instructions on how to shoot.
The video is definately 320x240x30fps and far better than any other SE Phone's video. Only the W900 comes close but its blurry compared to K850.
No lights photo (taken in a hurry too):
