Hi
I use
www.macoman.co.uk - they are very good. I was recommended them on a business forum and I now use this host exclusively for any site that I run

Well, to do that you're going to need either CGI support, ASP or PHP for the form handling script. Many webhosts offer a form-to-mail script pre-installed for you so keep an eye out for that.
Personally, I use
www.web-mania.com. £20 per year plus VAT for the hosting and it has full PHP/MySQL support.

cheers everyone, i think il go qith cyco's! very cheap! how much is 12gb a month? is it enough for a new site with not many visitors? can you easily buy more bandwidth if not enough? thanks mate

Yeah, 12Gb a month is more than enough for a general site, even a pretty popular one. If you're going to offer large file downloads (>100Mb each) or host lots of very high resolution photographs then you might need to up your limit, which you can do from your web-mania control panel.
Web Mania send you an email if you start getting near to your 12Gb limit, you can then order extra bandwidth if you wish. If you don't, access to the site will be suspended until the start of the next month once your limit has been used up.
