Here's the full guide:

Tools required: Torx 6 screwdriver, Phillips size 0 screwdriver
* First remove the battery cover, battery, memory card and stylus. Remove the three screws you see
* Run your fingernail around the join of the back cover and then pry the back cover off
* Your phone will now look like below. Now remove the two screws you can see at the top of the phone
* Now run your fingernail along the join of the front cover and you'll hear the clips, which hold the cover on, click. The front cover is on quite tight so use some force to pry the cover off
* With the front cover removed your phone will look like the picture below
* Remove the keypad and lift the part underneath from right to left, as shown below to reveal two more screws. Remove these screws
* Remove the metal plate that the screws were holding in place and you now have access to the ribbon which attaches the screen to the circuit board
* Detach the ribbon to remove the LCD screen. The ribbon attaches like a plug and socket. Simply lift the ribbon up away from the circuit board to detach it
* You have now removed the LCD screen. Reverse the whole procedure for reassembly

Great guide, now I just need to find a place that sells a replacement shell. Not too long ago they were all ove eBay, now I can't seem to find them anymore.
Anybody have a source?
