Could you clarify if you need to run the cab file TWICE as instructed in the Readme file? This seems strange but for people like me who connect my device to my PC to synchronise, it specifically seems to say the cab file should be run, then connect the device to the PC and then run the patch again. Am I misunderstanding something here??

I'm not a techie myself but have been on the receiving end of the fault this patch should be correcting. From what I understand it only affects phones with WM6.1 and results in a problem sending emails. Apparently if the phone comes across a problem connecting to the server it corrupts the email account settings resulting in the need to delete the account and setting it up again from scratch. It affected my HTC Touch Pro for 3 whole days (over a weekend of course!) but strangely seemed to correct itself after that.
Now if only someone would confirm how to install this patch.....

Dunno, I ran it only once, and no problems here.
@jazzywheelz; I am not an expert in WinMo, X1 is my first WinMo phone. But as a long time Windows user, I have usually updated my OS when a patch like this is released. Just in case...
Just download the CAB file, transfer it to your phone, and run from there.

My Windows Live email account seem unaffected, but I've had to delete and add my yahoo email account 3 times now! As it fails to work for no reason... or so I thought...
The emails error a few have mentioned are down to a good olde Windows Mobile 6.1 BUG! Yes that means "all" Win Mobile handsets are affected.
After re-adding my Yahoo account a couple of times, as a work around, I thought a "cleaner" and easier fix might be around. Well just tonight I've found this BLOG from someone else that seems to confirm the issue.
For those interested:

According to the author (Matt) he has had this BUG confirmed by Microsoft themselves. Apparently a fix is in progress and Matt will keep those interested informed.

I haven't had the send emails problem for quite a few weeks and after installing the patch a couple of days ago, this afternoon the problem returned. This patch is obviously not a patch after all! Come on Microsoft, you must do better. This is such a basic and fundamental problem which needs to be sorted immediately.
