Just a thought here, I got "Exception Exception homeflash.msconnector.a: Failed to get second Ack for loader header!!!" in the log when trying to update my phone but after trying on an older machine it worked like a charm. It didn't work on my XP SP2 with Athlon XP-M 3000 CPU but on an 18-month old P4 2,53 GHz also running XP SP2.
Maybe newer PC are just too damn fast (yeah I know, always slow when you need it to be fast)?
Some people were successful in upgrading when they tried on older machines, laptops or as one guy said that he didn't bother to close any other programs, and had a lot of explorer windows running and it worked for him.
If you have a laptop with CPU-speed control, try running it on power-save to get lower CPU-speed and on a desktop perhaps there is a CPU-slower utility.
Just my thoughts.
