Maybe because according to their own description, Total irremote has the ability to import device definitions and one is able to customize them to his / her own needs.
Ok the ui is the same on both programs, but paying 10 euro's more for 1 extra feature is quite a joke!

deliciosomarron is correct, when a device (tv, vcr, ect..) is not supported with ir remote, it let you import codes (found on internet) and install on the app.
