Do you have R2AL029? Is the Radio icon under entertainment? Did you use SE Tool or DaVinci?

Did you also notice that there is a strange shadow in dialog windows with an icon in them? I mean, if you switch profile for example, then there is a silly little shadow behind the icon in the window that pops up to tell you that you switched profile. This goes for all kinds of dialogs with an icon in them... please confirm.

Correct - I was fooled by looking at the pictures in SE Themes Creator 1.6... the radio icon occupies keypad number 9... that's weird!

Shadow? You mean if the keylock is activated, the dialogue box that popups has a shadow or the key within it has a shadow?

@govigov
It's hard to explain. If the theme has a dark background, then it's very noticable. The keylock icon is one of those icons that don't have a shadow though. This looks silly, and I don't think it was the case with R2AE033...
It's most noticable on the icon that shows in the window that pops up when switching themes. It's a light shadow under the paintboard...
The shadow looks silly since it seems transparent... hard to explain, but I havent seen it before...

When I still got my K7 last year, with the stock firmware on it, there were some ocassions that one of the ICONS was missing - (just forgot what was it), intermittently
