erm?
I can't say anything about ringtones you add to the phone because I haven't added any as I always use vibrate anyway.
On the second point I just tried it (by calling myself from another phone). They don't start quieter when increased ring option is on, they just get to full volume much slower.
I also noticed that there is a little toot toot and a standard type ring-ring before the ringtone itself starts.

i would rather that tones come in at your selected volume, and that the increasing ring is strictly an optional feature, i don't want to miss calls and i don't want to use vibrate all the time for battery reasons

lol! yeah I'm sitting here in a swealtering office in Frankfurt with my headset on listening to London's Capital Radio and they play that alot so it did make me laugh!
