Not really as long as you do a job that doesn't have much communication. Most everyone below 50 years of age speak English, it is a compulsary subject/class in school.
But some people have forgotten their school knowledge, they understand but they don't (dare) to speak.
Above 50 year old people: it depends on the person if he/she knows English. Many do, often because of their own interest. Persons born in 1947 were the first to get the opportunity to learn English in primary school.

As said, if you need to communicate with people/customers/patients you are required to speak Norwegian, Danish or Swedish.
So working at an elderly peoples' home wont be easy. But other jobs are available. But anyway, first you will have to get a working permit...
