Uh oh... I experienced the missing phone book entry syndrome with the R2E T68m(i) this afternoon. Good thing my trusty T39 was there to back-up my whole contact list.
Observation:
1. In the R2E firmware, one could not re-arrange the entries by first or last names and therefore, JWBlue's solution could not be applied.
2. In trying to edit all the blank entries, my business card would appear as the editable entry with some missing numbers.
3. All names and their respective numbers appear when trying to view the Position List (Phonebook > Advanced > Position List)
Instead of resetting the phone, I deleted all items in my T68's phonebook and used my T39's contact list to upload the data to the T68.
I shall observe further and report if the problem persists.
(This is my 4th T68, the 3rd was replaced because of the same problem as evidenced by my first post in this thread)

i hit my joystick-down and i could scroll through the contacts and see "balloons" at some lines, meaning that was a contact with a voice tag...
i tried editing a contact and all i saw in the form fields were "00"...
i tried adding a contact, and that was the only "visible" contact i had, though i still had my 200+ invisible contacts there.
i tried synching (using Outlook) but my contacts were still invisible.
so what i did was
delete all my contacts, and re-sync.
luckily now i could see them all again.

i have upgraded my phone recently to r6a. just this evening, i ran out of battery power while on the road. when i got back home and started charging, i noticed that i have several blank entries (no names, but with phone numbers "00") and several missing contacts (though their names still appear properly in text messages or calls.
does anyone have any solution for restoring these mysterious entries?

I got a solution that won't require you to fiddle with your phone settings.
I took out the battery for about 20 minutes and that seems to have "refreshed" the contacts list. I don't know how much time is really need, I just decided to try it out after reading some posts in a different thread that taking out the battery "resets" the phone.
Now it's working just fine!

This worked for me! Took care of the mystery blank entries and the missing contacts on my T Mobile T68i. Thanks a lot JWBlue!!

For those of you that experience this problem, I had the same thing happen several times.
I tried the sort order trick, which didn't work, but reading about the guy that had this problem happen as a result of taking out the battery while the phone was on gave me an idea.
Simply take out the battery while the phone is on. I turned on the phone again and every contact came magically back. I'm guessing it has something to do with the improper shutdown of the OS causing the contacts to go haywire, but this trick worked for me and it may work for others.
