Just come across this site and thought I would help you guys out!!
The GSM code 61 will work across all networks, however the networks make it difficult for you to deactivate CF because they earn revenue from it.
I too was also confused why my orange SPV M100 could not be configured to deactiave CF on no reply. The handset would basically accept the code , the network would reply "command successful", but CF on no reply was still active. The guys on the help desk were pretty useless, I knew that at their end my subscription details basically said do not allow disabling CF, or the the SIM toolkit was always morphing my deactivate command into an activate command.
The solution was:
Contact the operator ask for ALL CF services to be disabled. In my case this meant an SIM update was sent. This did either 1 or 2 things. Modify the SIM toolkit to prevent call morphing or changed my subscription details so that CF disablement was allowed. (although why that had to be sent as a SIM update I'm not sure).
So I can configure ALL CF services independently.
